Verify Your Benefits!

Here's how this process works:

1. You will enter your name and email below for our system. This ensures we know when you started this process so we can check in on your benefits verification.
2. You will be redirected to Berry Street, our service for verifying your benefits and scheduling an initial session. 
3. On Berry Street, you will enter your name, email, state, date of birth, and insurance information. 
4. You will then be  prompted to book an initial session with our team. Depending on your state, it is possible that some of our providers may not be listed, e.g. if you live in OR or WA, you will only see Autumn listed; if you live in CA, you will see most if not all of our providers listed. 
5. Within 48 hours, Berry Street verifies your benefits. Our team will reach out to you via email with your benefits information [# of covered visits, copays, deductibles, etc.].  
6. You attend your initial session as scheduled! Woohoo. If for any reason you decide not to move forward with your session or if you need to reschedule, you can always make changes.
